CAREinMIND Wellbeing Support Service
CAREinMIND is a low intensity counselling, information and referral service for people living, working or studying in the North Western Melbourne PHN catchment area.
Services provided:

CAREinMIND Suicide Prevention Service
CAREinMIND Suicide Prevention Service (SPS) provides the intake, assessment and allocation of referrals for clients who have a heightened risk of suicide. SPS is one component of a system of care that promotes a person-centred approach, matching support to the individual according to their specific needs.

- Referral, intake and assessment of clients
- Immediate counselling
- Allocations of low to medium risk clients to CAREinMIND providers
- Detailed call activity and service performance reporting.

MindHealth
MindHealth is a free professional phone and online counselling service operating between 7am and 9pm Monday to Saturday for people 15 years and older living or working in the Hunter New England and Central Coast regions.

- Professional online counselling – web chat between 7am and 9am Monday to Saturday

Service Connect Line
Service Connect Line is a free telephone service that links people with a mental illness residing in the Hunter New England and Central Coast Primary Health Network to psychosocial support services (mental and physical health as well as social supports). Carers, Clinicians and GPs can also call this line on a person’s behalf.

Region Access is a low intensity counselling, information and referral service for people living or working in the Country South Australia PHN catchment area.

What are Primary Health Networks (PHNs)?
The Australian Government is committed to delivering an efficient and effective primary health care system through the establishment of Primary Health Networks (PHNs).
In 2015, 31 PHNs were established to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and to improve coordination of care to ensure patients receive the right care in the right place at the right time. PHNs achieve these objectives by working directly with general practitioners, other primary health care providers, secondary care providers and hospitals to facilitate improved outcomes for patients.
